I am willing to pass my voting priviledges on to Babjak for my 102,000 shares. I am not sure how to proceed.

Do I enter her name on the appointee line, sign the signature block and mail it to the address in the supplied envelope?

Or do I just sign the form and forward it to her?

Or some other action?
